Mileage and Expense Reimbursement Form
All purchases must be cleared by Anne before using a personal form of payment. Reimbursement requests may be denied if the appropriate approval is not received before completing a purchase. Always ask your direct supervisor for a company card before using your own. Mileage reimbursement should also be approved by a supervisor before your work-related trip begins.

Incident Reports
Please notify your direct supervisor of an incident. There are different procedures depending on the details of the incident and if it is animal only, employee, or non-employee. Please follow these procedures to ensure the proper protocol is followed.
